What Is Pyrantel Pamoate?

Pyrantel pamoate is an antiparasitic medication belonging to a group of drugs called tetrahydropyrimidine anthelmintics. It is used to remove susceptible gastrointestinal nematodes—commonly called roundworms—from the digestive tract.

“Pyrantel” refers to the active drug, while “pamoate” refers to the salt form used to produce a poorly water-soluble medication. This formulation is absorbed only to a limited extent from the gastrointestinal tract, allowing much of the drug to remain where many target worms are located.

Why Accurate Parasite Identification Matters

Diarrhea, poor weight gain, vomiting, abdominal enlargement, anemia, and visible worms can occur with several different infections. Treating every suspected parasite with pyrantel may delay the correct diagnosis.

A fecal examination can help identify:

Pyrantel Pamoate vs. Pyrantel Base: Why the Difference Matters

Pyrantel pamoate and pyrantel base are related but are not numerically interchangeable.

Pyrantel base represents the amount of pharmacologically active pyrantel. Pyrantel pamoate represents the complete salt, which includes both pyrantel and pamoate. The salt weighs more than the active pyrantel portion.

Many veterinary dosage recommendations are expressed in milligrams of pyrantel base per kilogram of body weight. Product labels may list:

This difference can produce a serious calculation error. A person who interprets 144 mg of pyrantel pamoate as 144 mg of pyrantel base may miscalculate the volume substantially.

Before calculating any liquid dose, confirm:

  1. Whether the recommendation is expressed as pyrantel base or pyrantel pamoate salt.

  2. The animal’s accurate current weight.

  3. The number of milligrams of pyrantel base in each milliliter.

  4. Whether the product contains additional active ingredients.

  5. The labeled species and age restrictions.

  6. The veterinarian’s prescribed treatment interval.

Never copy an mL dose from another bottle, website, animal, or previous prescription unless the concentration and clinical instructions are identical.

Available Pyrantel Pamoate Forms and Concentrations

Pyrantel pamoate is available in several formulations, including:

The most important difference between products is not their flavor, package size, or brand name. It is the amount of pyrantel base contained in each milliliter or tablet.

In the United States, regulated pyrantel pamoate suspensions may contain the equivalent of 2.27 mg, 4.54 mg, or 50 mg of pyrantel base per mL. These concentrations are not interchangeable. One milliliter of a 50 mg/mL suspension contains more than ten times as much pyrantel base as one milliliter of a 4.54 mg/mL suspension.

Some labels state the concentration as “pyrantel pamoate equivalent to” a certain amount of pyrantel base. Others may prominently display the weight of the pamoate salt. This can make two products appear similar even when they provide different amounts of active pyrantel.

For example, a human pinworm suspension listed by DailyMed contains 250 mg of pyrantel base in 5 mL, equivalent to 50 mg/mL. Its human dosing directions must not be applied automatically to a dog or cat.

Before calculating any dose, identify all three of the following:

  1. The animal’s current and accurate weight

  2. The prescribed amount of pyrantel base per kilogram or pound

  3. The product’s pyrantel-base concentration in mg/mL or mg/tablet

Never assume that two bottles labeled “pyrantel pamoate” should be administered at the same volume.

The US regulation for certain canine pyrantel pamoate suspensions specifies a single oral dose equivalent to 2.27 mg of pyrantel base per pound of body weight, which is approximately 5 mg/kg. The labeled indications include susceptible canine roundworms and hookworms. The regulation also notes that additional treatment may be necessary and recommends confirming the result with a fecal examination within two to four weeks.

The basic calculation is:

Required pyrantel base (mg) = body weight (kg) × prescribed dose (mg/kg)

The required liquid volume is then calculated separately:

For example, if a veterinarian recommends 5 mg/kg of pyrantel base for a 10 kg dog:

  • Required pyrantel base: 10 kg × 5 mg/kg = 50 mg

  • With a 50 mg/mL suspension: 50 ÷ 50 = 1 mL

  • With a 4.54 mg/mL suspension: 50 ÷ 4.54 = approximately 11 mL

The required active dose is identical, but the liquid volumes are dramatically different.

This example should not be treated as an individual prescription. Some combination products use different dosing systems, fixed tablet weight ranges, or additional active ingredients that determine how the product must be administered. Always follow the veterinarian’s directions and the label belonging to the exact product.

Very small volumes may require a precisely marked oral syringe. Do not estimate them with a kitchen spoon. Shake liquid suspensions as instructed, measure the dose carefully, and confirm whether the medication should be repeated. The appropriate schedule depends on the dog’s age, infection risk, fecal-test findings, and the exact product used.

Pyrantel Pamoate Dosage for Cats

There is no single pyrantel pamoate volume that is appropriate for every cat or every product. The correct dose depends on:

  • The cat’s current weight

  • The parasite species being treated

  • The amount of pyrantel in the product

  • Whether pyrantel is used alone or with another dewormer

  • The cat’s age, health, pregnancy status, and previous treatments

This distinction is especially important because many feline products combine pyrantel pamoate with praziquantel or another antiparasitic drug. Their tablets are dosed according to the complete formulation—not according to pyrantel alone.

For example, the FDA-approved US label for Drontal for cats and kittens provides weight-based tablet directions for a combination of praziquantel and pyrantel pamoate. It is indicated for certain tapeworms, hookworms, and roundworms. The label states that the cat should be weighed before treatment and that the product should not be used in kittens younger than two months or weighing less than 2 lb.

These directions must not be transferred to another brand or formulation. A liquid containing pyrantel alone may require a completely different volume, while another combination tablet may contain different amounts of every active ingredient.

Veterinary guidance is particularly important when treating:

  • Very young or underweight kittens

  • Sick, dehydrated, or severely anemic cats

  • Pregnant or nursing cats

  • Cats receiving other parasite medications

  • Cats with recurrent diarrhea or repeated positive fecal tests

  • Cats whose parasite species has not been identified

A fecal examination can help determine whether pyrantel is suitable. Giving pyrantel repeatedly without identifying the parasite may delay effective treatment because many causes of feline diarrhea—including Giardia, coccidia, tapeworms, and non-parasitic diseases—are not controlled by pyrantel alone.

The label states that the tablets may be administered directly by mouth or offered in a small amount of food. Food does not need to be withheld before or after treatment. It also advises consulting a veterinarian before administering the product to a sick or pregnant cat.

This table is not a universal pyrantel chart. Do not use the same number of tablets from a different product simply because it also contains pyrantel pamoate. Likewise, do not convert the tablet count into a dose of human or canine pyrantel suspension.

Cats falling between marked syringe volumes or outside a product’s labeled weight range require veterinary guidance. Estimating by eye can produce a meaningful dosing error, particularly in kittens.

How to Calculate Pyrantel Pamoate Liquid Dosage Correctly

Liquid pyrantel dosage should be calculated in two separate stages. First calculate how many milligrams of pyrantel base are required. Then convert that amount into milliliters using the exact concentration printed on the bottle.

Step 1: Record the animal’s weight

Use a recent, reliable weight in kilograms. To convert pounds to kilograms:

Weight in kilograms = weight in pounds ÷ 2.2

For example:

8.8 lb ÷ 2.2 = 4 kg

Step 2: Confirm the prescribed dose

The veterinarian’s instruction should identify the intended dose in mg/kg or mg/lb and clarify whether it refers to:

  • Pyrantel base

  • Pyrantel pamoate salt

  • A particular commercial product

Never assume that “pyrantel 10 mg/kg,” for example, means the same thing as “pyrantel pamoate 10 mg/kg.” The base and salt weights are different.

Step 3: Calculate the required milligrams

Required dose (mg) = body weight (kg) × prescribed dose (mg/kg)

If a veterinarian prescribed 5 mg/kg of pyrantel base for a 4 kg animal:

4 kg × 5 mg/kg = 20 mg pyrantel base

Step 4: Find the concentration on the label

Look for wording such as:

The animal receives the same number of milligrams in each example, but the measured volumes differ by more than twentyfold. US regulations list pyrantel pamoate suspensions with concentrations equivalent to 2.27, 4.54, or 50 mg of pyrantel base per mL, demonstrating why instructions copied from another bottle can be dangerous.

These examples demonstrate the calculation method; they are not dosing recommendations for an individual dog or cat. Have the veterinarian verify the chosen dose, concentration, and final volume before administering the medication.

How to Give Pyrantel Pamoate to Dogs and Cats

Pyrantel pamoate is administered orally. Depending on the product, it may be supplied as a liquid suspension, tablet, chewable tablet, or paste.

Before giving the medication:

  1. Weigh the animal accurately.

  2. Confirm the prescribed dose.

  3. Check the product’s concentration and active ingredients.

  4. Read the label for storage and preparation instructions.

  5. Use an oral dosing syringe for liquid medication.

Liquid suspensions should be shaken thoroughly when instructed. The active ingredient can settle during storage, so drawing medication from an unshaken bottle may result in an inaccurate dose.

To administer a liquid:

  1. Measure the exact volume with an oral syringe.

  2. Approach the animal calmly and avoid forceful restraint.

  3. Place the syringe into the side of the mouth, behind the canine tooth.

  4. Direct the liquid across the tongue rather than straight toward the throat.

  5. Administer it slowly, allowing time to swallow.

  6. Confirm that the medication was not immediately spit out.

Do not tilt the animal’s head sharply backward or rapidly inject liquid into the throat. Doing so can increase the risk of coughing, choking, or aspiration.

Tablets may be administered directly or hidden in a small amount of food when permitted by the product label. Use only enough food to conceal the medication; mixing it into a full meal makes it difficult to confirm that the entire dose was consumed.

If the animal spits out part of the dose or vomits shortly after administration, do not automatically give another full dose. The amount absorbed or swallowed may be uncertain, and repeating it could cause an excessive total dose. Contact the veterinarian for instructions.

Some pyrantel products contain additional active ingredients. Never crush, divide, or substitute a combination tablet unless its label permits division and the resulting portion supplies the correct amount of every ingredient.

Should Pyrantel Pamoate Be Given With Food?

Pyrantel pamoate can often be given with or without food, but the directions for the exact product should always take priority.

US regulations for certain canine pyrantel suspensions state that the single dose may be placed in the dog’s feed bowl either by itself or mixed with a small quantity of food.

The FDA-approved Drontal label for cats and kittens similarly states that the tablets may be given directly by mouth or offered in a small amount of food. Food does not need to be withheld before or after treatment.

Giving the medication with a small meal may be helpful when an animal:

  • Resists direct oral medication

  • Experiences mild nausea with medications

  • Accepts tablets more readily when hidden in food

  • Needs a less stressful administration method

However, the entire dose must be consumed. Avoid placing the medication in a large bowl of food that the animal may not finish.

Fasting, laxatives, enemas, and dietary restriction are generally unnecessary unless a veterinarian has recommended them for another medical reason. Do not withhold food from very young, underweight, diabetic, or medically unstable animals without veterinary instructions.

A single dose may remove susceptible adult worms currently present in the intestine, but it may not eliminate every immature stage or prevent reinfection. For this reason, repeat treatment is sometimes necessary.

  • The degree of environmental contamination

  • Whether other animals in the household are infected

  • The results of follow-up fecal testing

  • The animal’s ongoing exposure risk
